• Document type (e.g., saddle, septic)
• Document if cor pulmonale is present. If so, document if cor pulmonale is acute or chronic.
• Specify if PE is chronic (i.e., still present) or resolved. Do not use "history of pulmonary embolus."
• Document if patient is taking anti-coagulant therapy for treatment of active pulmonary embolus or prohylaxis.
